Pediatric Nursing

Pediatric #nursing is the #medical #care of #neonates and #children up to #adolescence, usually in an in-patient hospital or day-clinic. A technique called "#electrical #grounding" may moderate #preterm #infants' electromagnetic exposure in the #neonatal #intensive #care unit (NICU) and improve their health outcomes. Equipment in the #NICU produces low-frequency electromagnetic fields that can have subtle yet measurable effects on the autonomic nervous system, the system that regulates involuntary body functions. #Preterm #infants may be especially vulnerable to these effects.

Previous research in adults has shown that exposure to electromagnetic fields can affect the vagus nerve, a key component of the autonomic nervous system which regulates the body's internal organs during rest. Previous research also has shown that electrical grounding, which reduces the electrical charge to the body, can improve the functioning of the autonomic nervous system and the vagus nerve, producing improved vagal tone.
Vagal tone, which is measured by analyzing heart rate variability between inhalation and exhalation, is a valuable indicator of health. An earlier study performed with colleagues at Penn State found that low vagal tone in preterm infants is a marker of vulnerability to stress and a risk factor for developing necrotizing enterocolitis, an intestinal disorder that can have severe consequences. Strengthening vagal tone may reduce inflammation, guard against the development of necrotizing enterocolitis and offer protection from a variety of other conditions that can affect preterm infants.

Human rhinovirus (HRV), the culprit behind most colds, is the leading cause of hospitalization for premature babies. However, in very preterm children, exactly how HRV causes severe respiratory disease -- and which patients may need more intensive observation and treatment -- is less well understood.
A new study led by Children's National Health System research-clinicians showed in children who were born severely premature, HRV infections seem to trigger an airway hyper-reactivity (AHR) type of disease, which leads to wheezing and air-trapping (hyperinflation) and more severe respiratory disease. This, in turn, increases the risk for hospitalization.
It is found that other signs of respiratory distress, such as low arterial blood oxygen or rapid shallow breathing, were no more common in severely premature children (less than 32 weeks of gestational age) than in kids born preterm or full-term. The findings have implications for administering supportive care sooner or more intensively for severely premature children than for other infants.

Researchers from Children's Minnesota (Children's) and HealthPartners Institute have developed a new pediatric appendicitis risk calculator (pARC) to aid in the diagnosis of appendicitis. The calculator was developed as part of a $3.1 million, five-year grant from the National Institutes of Health (NIH).
In the April issue of Pediatrics, the researchers describe a novel method to calculate a patient's specific risk for appendicitis. With this new method, clinicians will be able to provide tailored medical and surgical guidance to a patient. In the study, researchers used data collected from ten pediatric emergency departments to develop the risk calculator and then independently validated the score using data from a single children's hospital. Potential benefits of using the new risk calculator include a reduction in the use of computed tomography (CT), more judicious utilization of ultrasound and a reduction in healthcare expenditures.
"This method is of great benefit to our patients and the health care system overall," said Dr. Anupam Kharbanda, co-Principal Investigator and chief of critical care services at Children's Minnesota. "In addition to being able to target our care specifically to each patient, we're also reducing the use of unnecessary medical tests and expenses. We're thrilled to have developed a new way to standardize care for children and adolescents with abdominal pain."
